Different Name on Notice of Intended Marriage than Birth Certificate

Where a party wants to use a different name on the Notice of Intended Marriage to the name recorded on their birth certificate, they should apply to Births, Deaths & Marriages in the State or Territory where they were born or where they live for a change of name certificate to be issued to them.
